New Patients:
Registration, medical history and other forms can be sent to you after you fill and return a consent for electronic transmission. We also need Vanderbilt forms from parents and teachers on school-age patients. Please send us a copy of your insurance card (both sides) if you are using insurance and present to our office 20-30 minutes before your appointment.
Established Patients:
Please check in 10-15 minutes before your appointment to make a payment and update your address and pharmacy, we also need to be notified before your visit if your insurance has changed and please remember: patient portal is the best way to communicate with your doctor.
Communications via email over the internet are not secure, there is a possibility that information you include in an email can be intercepted and read by other parties besides the person to whom it is addressed. We do not recommend that patients use email to send or receive private health information. Patients who elect to use emails accept the risks associated with the use of unsecure E-mail communications. As with all means of electronic communication, there may be instances beyond the control of the family and the health care provider where information may be lost or inadvertently exposed, such as during technical failures, acts of God, acts of war, and so forth. Patients who ask to use email to communicate with ADHD Houston acknowledge the privacy risks associated with that usage.
